Complete Description:

POSITION PURPOSE: Member of the LTC Facility interdisciplinary health team who oversees the LTC Social Service and Activity Programs. This individual assures that the residents have a primary advocate and that each resident is able to meet his/her psychosocial needs and to have a high quality of life. In addition this person works to assure that each resident is provided activities of preference that are appropriate to individual and group needs and interests of each resident, promoting opportunities for engagement in normal pursuits. The individual in this position works under the direction of the Director of LTC, and working with the nursing staff, strives to assist residents to maintain and/or regain their optimal state of activity, or to provide psychosocial support and comfort to the resident and their family.

KNOWLEDGE AND EXPERIENCE:

  1. Must be at least twenty-one (21) years of age.
  2. Must possess a MSW or related degree.
  3. One year (1) Social Work experience in geriatric extended care facility is required.
  4. Maintains current BLS certification.
  5. Knowledge of local and State agencies and services provided, is preferred.
  6. Knowledge of Federal and State regulations pertaining to LTC is required.
  7. Technical writing skills and/or knowledge of Policy/Procedure development preferred.
  8. Knowledge and understanding of Resident Rights and Abuse Prevention required.
  9. Current Alaska drivers license with ability to drive the LTC van. Able to be accepted on SPH driving insurance.
  10. Speaking, listening and basic writing skills required to effectively communicate with others and to document care provided. Ability to complete computer documentation as required.
  11. Must complete general orientation for Long Term Care.
